Students will be awarded 0.25 credits for Term 2 on a pass/fail basis by completing the service requirements. Please return this completed verification form to your Theology Teacher on the first Theology Class day on or following February 1, 2011. Meeting the deadline, properly completing the form, doing appropriate service and having all necessary verification information will insure that a student earns a passing grade. After February 1, you will not be permitted to take part in school social activities until you return a completed service form and it is verified. You will also be given 1 demerit for each day your paper is late.

DIRECTIONS

1. Your service may be done for any non-profit organization. A non-profit has tax-free status and has an IRS 501©3 number. Non-profits usually fall into one of the following groups: arts, culture, humanities; education; environment and animals; health; human services; international, foreign affairs; social welfare and advocacy; religion related; charities; and community or school athletic programs.
2. You may only count ten (10) hours of service to MPS. Service done during school hours as part of a class cannot be counted.
3. Service done for an individual person or for a member of your family must be approved by the MPS Service Coordinator before you do that service. Babysitting is NOT community service.
4. You can create you own service project if you have the permission of the school service coordinator.
5. It is important to fill in all sections of this form. Get a verification signature at the time you do your service. Please print the supervisor’s name and phone number clearly. A letter from your supervisor on agency letterhead may be substituted for the signature. A letter is suggested for verification of out-of-town service.
6. Volunteering to work without pay for a regular business (i.e. dentist office, auto dealership, bank, retail stores etc.) cannot be counted as community service for any reason.
